In order for your child to attend Beverly City School, they must first register.

Forms necessary for registration

1. Completed registration form 
2. Health examination form
3. Early screening parent questionnaire (Pre-kindergarten and Kindergarten)

IMPORTANT...  The following documents must accompany completed forms

1. Birth Certificate
2. Proof of Residency (One of the following is Mandatory)
   ​Deed, Mortgage Statement, Lease/ Rental Agreement
3. Three Forms of Supporting Documentation of Residency
   (Recent utility bills, voter’s registration card, recent tax bill, homeowner’s Insurance Card or Credit Card bill with home address, Driver’s License)
4. If living with a Beverly resident a Notarized Affidavit and additional proof will be required.
5. Record of All Required Immunizations
   In accordance with state law, children must have all the required immunizations and a record of physical examination.

* If registering a student that is transferring from another district a transfer card and all transfer paperwork is required.
 
* If the student has an active IEP, please bring a copy with you to registration.
