NJ immunization and vaccine requirements

We are required to ensure compliance with immunizations per NJ State Regulations. Anytime your child receives a vaccine, please submit proof to the school nurse so that I can update his/her health file. I have included the vaccine requirements from the NJ Department of Health for your review under the important links list.

Students entering the 6th grade

​The State of New Jersey requires that all children entering sixth grade receive a dose of both the Tdap and meningococcal vaccines, PRIOR TO THE FIRST DAY OF SCHOOL. I am writing to notify you of this requirement so that you have ample time to ensure compliance with this mandate. A physical is also recommended. Please provide me with documentation that your child has received these two vaccines as soon as they are completed. In addition, please review my website for helpful fact sheets from the State about HPV and Meningitis. 

If your child already received their shots

Some of your children may have already received these shots and if that is the case, I simply need the documentation. If your child has a summer birthday, please provide me with the immunization record once he/she receives the vaccines. 

Children with their eleventh birthday in September

If your child’s eleventh birthday is in September, I will need an appointment date and time in order for him/her to attend the first day of school. If you do not present compliance as outlined above, your child may not start school in September. ​
