Athletic participation
Physical and health historyBefore a student can participate in athletics, he/she must have a physical exam. If the examination was completed more than 90 days prior to the first practice, the parent/guardian must also complete a health history update.
Concussion and head injuryAlso, the Beverly City Board of Education adopted a policy concerning the prevention and treatment of sports-related concussions. Below is the NJ Department of Education Concussion and Head Injury Fact Sheet. You must return the parent/guardian acknowledgement form before your child can participate.
Cardiac arrestIn addition, below is the educational brochure that describes basic facts surrounding sudden cardiac death in young athletes. Please review and return the completed sign off sheet.

Opioid use and misuseLastly, New Jersey law requires us to share with you an Opioid Use and Misuse Educational Fact Sheet. Please review the fact sheet, then sign/have your child sign and return the corresponding acknowledgement form.
